ROOT CANAL THERAPY

Root canal therapy is made necessary when a cavity reaches all the way to the root of a tooth. (Regular cleanings and checkups prevent and detect problems early) Sometimes deep restorations or trauma to a tooth may cause the nerve to be damaged to the point it needs root canal therapy, also. Once this occurs the pulp becomes infected, and can even extend through the root tip and begin to eat away at the surrounding bone (this is an abscess). By the time the pulp is infected it must be treated, and cannot heal on its own. It can even weaken the entire immune system. This is dangerous, not to mention very painful. Symptoms that the pulp has become infected may include sensitivity to hot/cold or sweets, pain, swelling, pain to biting or pressure, and a bad taste in the mouth. Sometimes, however, no symptoms are apparent and the person is unaware of any problem until a checkup.

ROOT CANAL RETREATMENT

With proper care, even teeth that have had root canal treatment can last a lifetime. But sometimes, a tooth that has been treated doesn't heal properly and can become painful or diseased months or even years after treatment. If your tooth failed to heal or develops new problems, you have a second chance. An additional procedure may be able to support healing and save your tooth. If you are experiencing dental pain or discomfort in a previously treated tooth, talk to an endodontist about retreatment.
As occasionally happens with any dental or medical procedure, a tooth may not heal as expected after initial treatment for a variety of reasons:

  • Narrow or curved canals were not treated during the initial procedure.
  • Complicated canal anatomy went undetected in the first procedure.
  • The placement of the crown or other restoration was delayed following the endodontic treatment.
  • The restoration did not prevent salivary contamination to the inside of the tooth.
In other cases, a new problem can jeopardize a tooth that was successfully treated. For example:

  • New decay can expose the root canal filling material to bacteria, causing a new infection in the tooth.
  • A loose, cracked or broken crown or filling can expose the tooth to new infection.
  • A tooth sustains a fracture.

APICOECTOMY

Your teeth are held in place by roots that extend into your jawbone. Front teeth usually have one root. Other teeth, such as your premolars and molars, have two or more roots. The tip of each root is called the apex. Nerves and blood vessels enter the tooth through the apex, travel through a canal inside the root, and into the pulp chamber, which is inside the crown (the part of the tooth visible in the mouth).

An apicoectomy may be needed when an infection develops or persists after root canal treatment,or retreatment. During root canal treatment, the canals are cleaned, and inflamed or infected tissue is removed. Root canals are very complex, with many small branches off the main canal. Sometimes, even after root canal treatment, infected debris can remain in these branches and possibly prevent healing or cause re-infection later. In an apicoectomy, the root tip, or apex, is removed along with the infected tissue. A filling is then placed to seal the end of the root. An apicoectomy is sometimes called endodontic microsurgery because the procedure is done under an operating microscope.

ROOT AMPUTATION

Root amputation is a specialized dental procedure, whereby one root is removed from a multi-root tooth. The tooth is then stabilized and rendered fully functional with a crown or filling. The multi-root teeth best suited to the root amputation procedure are the molars at the back of the mouth. These large flat teeth have either two or three roots depending on whether they are situated on the upper or lower jaw.
The general purpose of root amputation is to save an injured or diseased tooth from extraction. Most dentists agree that there is no better alternative than retaining a healthy natural tooth, and the root amputation procedure makes this possible. Dental implants, extensive bridgework and custom-made tooth replacements can be expensive and time-consuming. Generally, root amputation and the necessary crown work are less expensive and can be completed in 1-3 short visits.

When is root amputation necessary?
It is important to note that root amputation can only be performed on an otherwise healthy tooth. Even in the case of a “key” tooth, extraction will be performed if the tooth is diseased, badly fractured or otherwise injured. Suitable teeth for root amputation have a healthy tooth surface, strong bone support and healthy underlying gums.
There are several problems that may lead to root amputation including:
  • Broken, fractured or injured teeth and roots.
  • Embedded bacteria within the structure of the root.
  • Severe bone loss in a concentrated area due to periodontitis.
  • Tooth decay in a concentrated area of the tooth.

CRACKED TOOTH

There are many different types of cracked teeth. Cracked teeth show a variety of symptoms, including erratic pain when chewing, possibly with release of biting pressure, or pain when your tooth is exposed to temperature extremes. In many cases, the pain may come and go, and your dentist may have difficulty locating which tooth is causing the discomfort. If you are experiencing these dental symptoms or suspect a cracked tooth, see an endodontist, who specializes in saving cracked teeth.

The treatment and outcome for your tooth depends on the type, location and extent of the crack. If you think you have a cracked tooth, it's important to seek treatment quickly, before the problem gets worse. Once treated, most cracked teeth continue to function and provide years of comfortable chewing.

DIGITAL IMAGES

Dental digital images are a useful diagnostic tool when helping your dentist detect damage and disease not visible during a regular dental exam. How often digital images should be taken depends on your present oral health, your age, your risk for disease, and any signs and symptoms of oral disease. For example, children may require digital images more often than adults because their teeth and jaws are still developing and their teeth are more likely to be affected by tooth decay than those of adults. Your dentist will review your history, examine your mouth and then decide whether or not you need digital images.

Dental digital images exams are safe; however, they do require very low levels of radiation exposure. If you are a new patient, the dentist may recommend digital images to determine the present status of your oral health and have a baseline to help identify changes that may occur later. A new set of digital images may be needed to help your dentist detect any new cavities, determine the status of your gum health or evaluate the growth and development of your teeth. If a previous dentist has any radiographs of you, your new dentist may ask you for copies of them. Ask both dentists to help you with forwarding your digital images.

CBCT IMAGING

Cone beam computed tomography (or CBCT, also referred to as C-arm CT, cone beam volume CT, or flat panel CT) is a medical imaging technique consisting of X-ray computed tomography where the X-rays are divergent, forming a cone.

CBCT has become increasingly important in treatment planning and diagnosis in implant dentistry, interventional radiology (IR), among other things. Perhaps because of the increased access to such technology, CBCT scanners are now finding many uses in dentistry, such as in the fields of endodontics and orthodontics. Integrated CBCT is also an important tool for patient positioning and verification in image-guided radiation therapy (IGRT).

PUPAL REGENERATION

Regenerative endodontics is the use of biologically based procedures designed to replace damaged tooth structures such as dentin, root structures and cells of the pulp-dentin complete. Regenerative endodontics, is the extension of root canal therapy. Conventional root canal therapy cleans and fills the pulp chamber with biologically inert material after destruction of the pulp due to dental caries, congenital deformity or trauma. Regenerative endodontics instead seeks to replace live tissue in the pulp chamber.

To replace live tissue, either the existing cells of the body are stimulated to regrow the tissue native to the area or bioactive substances inserted in the pulp chamber. These include stem cell therapy, growth factors, morphogens, tissue scaffolds and biologically active delivery systems.
